Please document his actions and attitudes

David is on a track to potential discharge in March of 2016.  I won’t go in to the ins and outs of his program. I don’t necessarily agree with discharge at this point. David has been having difficulties the last two days.  And a couple of times over the last month. If his care and issues aren’t properly documented, my fear is that he will not get the care that he needs.  His behaviors are escalating. Please, don’t do him any favors by not documenting his care.  If he does something, please document it.  It is the only way to ensure that he is getting the care he needs. We saw this in his last placement where things weren’t documented in the lodge/cottage/residence.  He ended up being discharged against doctors orders because Medicaid said he needed to go home since he wasn’t making any progress and was not regressing either.

Psych stay again

Working on getting David admitted to psych again. He is throwing up his meds, stealing and refusing meals. He has lost 5 pounds in the last week due to throwing up his meds.  Tweet #fighting4answrs

>Rough day!!!

>Well, M is finally asleep. It has been a rough day. Started with him getting a bath and writing all over the tub with a pencil. Then the mouth started. Took away all privileges. Mouth continued. Ended up being a long day and he got incredibly mouthy. He got so mad he threatened to hurt himself. So we restrained him. Then he started cussing me out. Got him calmed down. Still no privileges. He was ok for a while, then we discovered that he had a book and paper that he wasn’t supposed to have. Took them away, and he started throwing things. Tried to throw my laptop. Was successful in getting it away from him without hurting him or the laptop. Got him calmed down again. I had to run an errand so left for a bit. We let him work on a puzzle with his siblings and mom.
